Blank Registration Form | | | | | |
If you never received a registration form from
you child's school you can print out a
blank form here. After the form is filled out please cut
the bottom portion off, attach the $35 fee for the class, and
either mail it (follow directions on the form) or have your
child bring it to the first day of class.
When filling out the form, please make sure to fill in all
the yellow highlighted blank lines. You can obtain the dates, time and day of
the class by calling (619) 582-1897 to speak with an Instructor. Click
